What can a Community Health Worker(CHW) do for me?
CHW’s act as a “bridge” connecting women and their families to programs, agencies, or doctors who can provide health services, screening, and interventions to help improve health outcomes. This is done in a language women can understand from knowledgeable workers who are familiar with their community. CHWs are a one on one peer support for women and their families. The CHW will be providing on-going follow –up and assessment of your needs. If you are between the ages of 15-44 years old and live in Monmouth or Ocean Counties, you can calls us, we can assist you!
